Fill out the form below to enter the Book Giveaway Contest.

Step 1: DO NOT write you last name in the box. Instead, write your teacher's last name.  For example, if your name is Jane and your teacher is Mrs. Hall, you would write:

First Name: Jane     Last Name: Hall  (if another student in your class has the same first
                                                                      name as you, add the first letter of your last name.
                                                                      For example, Janed)
